Thresholds are where each tier starts (single / joint, modified AGI from the 2024 return); the top tier starts at $500,000 single / $750,000 joint. Part B column is the total monthly Part B premium; Part D column is the surcharge added to the plan premium. Projected thresholds from CPI indexing (the $500,000 / $750,000 top tier is fixed by law through 2027); surcharge dollar amounts arrive with the Part B announcement. Married filing separately uses its own schedule. You can appeal (form SSA-44) after a life-changing event such as retirement, death of a spouse or divorce.

Hold-harmless and the COLA
The hold-harmless rule stops a Part B increase from cutting your net check when the COLA is small. With a 3.6% COLA on a $2,095 average benefit (+$75 a month) and a projected Part B rise of about $6.60, almost everyone keeps a net increase this year, so hold-harmless barely bites. IRMAA payers are never protected by it.
